Lemon juice (~ 30 ml) provides ~ 1.2 g citric acid, which is metabolised to bicarbonate and excreted in urine as citrate. Higher urine citrate binds calcium and prevents crystals from aggregating into stones.

Clinical trial evidence (Penniston, Seltzer, others) shows lemonade therapy raises urine citrate by ~ 30% and is associated with reduced calcium-oxalate stone recurrence — most effective in patients with documented hypocitraturia (low urine citrate). It is NOT as potent as prescription potassium citrate, but is a useful adjunct, cheap and well-tolerated.

There are NO trials showing lemon water improves eGFR, reduces proteinuria or slows CKD progression in non-stone-formers.

Lemon water and CKD — practical use

Most CKD patients should aim for adequate hydration (usually 1.5–2 L/day unless fluid-restricted on dialysis). Plain water is the gold standard but many people struggle to drink it. Adding lemon (and optionally cucumber, mint or ginger) makes plain water easier to drink without adding salt, sugar or potassium.
