Why apples are a renal-diet staple
Apples sit firmly in the 'lower-potassium fruit' group used by UK renal dietitians (alongside berries, pears and grapes). They:
- Add fluid in a satisfying form for those on fluid restriction.
- Contain pectin — a soluble fibre that gently lowers LDL cholesterol and slows glucose absorption.
- Provide vitamin C without potassium load.
- Are widely available, cheap, portable and don't need cooking.
Whole apple vs apple juice
A medium apple (~ 80 kcal, 4 g fibre, 19 g sugars but with fibre buffering absorption) is a slow-release fruit.
200 ml apple juice contains: ~ 22 g sugar, 0 g fibre, ~ 250 mg potassium, and raises blood glucose and uric acid sharply. In CKD this matters because:
- Higher uric acid is linked to faster eGFR decline.
- Sugar drives blood-pressure rises and weight gain.
- Concentrated potassium adds up if you drink several glasses.
If you enjoy juice, keep to 150 ml with a meal — and treat it as one of your daily sugar allowances, not as a fruit serving.

Portion guidance
- Stage 1–4: 1–2 medium apples per day, no restriction.
- Stage 5 / dialysis: 1 medium apple usually fine; check with your renal dietitian if K is persistently high.
- Stewed / baked apples: same nutrition; avoid added sugar and honey if you have diabetes.
- Dried apple rings: concentrated potassium and sugar — count one small handful (~ 30 g) as a fruit portion.
Practical CKD-friendly ideas
- Slice into porridge oats with cinnamon (no added sugar).
- Pair with a small piece of cheese or peanut butter for a balanced snack.
- Bake with cinnamon for a low-K pudding alternative to fruit crumble with high-K rhubarb or apricots.
- Choose Gala, Pink Lady, Braeburn — naturally sweeter, no need for added sugar.
- Avoid 'apple-flavoured' drinks and squashes — these are sugar/acid solutions with little real apple.
